Physical Examinations and Medical Release Forms
Students registering for Athletics must have a current physical examination and a medical release form on file before the student is authorized to participate in a sport. To ensure clearance for participation, both forms should be completed and turned in no later than 3 school days prior to the first day of tryouts.
Physicals and Medical Release Forms must be renewed each school year and are valid for the current school year only.
Once the forms are on file they do not need to be turned in again for another sport.
